Thao Hoang, FNP-C

Thao Hoang is a board-certified Family Nurse Practitioner providing comprehensive care in Family Medicine. She cares for patients across the lifespan and is dedicated to delivering evidence-based, patient-centered care tailored to each individual’s needs. As a bilingual provider, Thao speaks both English and Vietnamese and is committed to delivering culturally competent care to diverse communities.

Thao’s clinical interests include preventive care, women’s health, and the management of chronic conditions such as hypertension, diabetes, thyroid disorders, and metabolic health. She emphasizes patient education, shared decision-making, and preventive strategies to support long-term wellness.

Thao earned her Master of Science in Nursing from the University of Texas Medical Branch and her Bachelor of Science in Nursing from Cizik School of Nursing at UTHealth Houston. She also completed the CLS Health's Advanced Practice Provider Residency Program, where she gained hands-on training in outpatient primary care. Her clinical experience includes family medicine, internal medicine, pediatrics, and women’s health. Prior to becoming a nurse practitioner, Thao worked as a Registered Nurse in medical-surgical and women’s health settings at Memorial Hermann.

Known for her warm and approachable style, Thao values building meaningful relationships with patients and their families. She strives to create a welcoming environment where patients feel heard, respected, and empowered to take an active role in their health.
